Job summary

Trinity Health Of New England is seeking an Oncology Social Worker (LCSW) for our Outpatient Oncology & Infusion Center in Hartford, CT. You will conduct psychosocial assessments, provide counseling, and coordinate care to support patients through diagnosis, treatment, survivorship and end-of-life considerations.

Qualifications

• MSW from a CSWE-accredited program is required.
• Connecticut Social Work licensure is required; LCSW preferred or LMSW considered based on experience to obtain licensure.
• Minimum of two years of healthcare/social work experience in oncology/infusion/ambulatory care or related setting preferred.

Responsibilities

• Perform psychosocial assessments and distress screenings for oncology patients and families.
• Provide individual, family, and supportive counseling related to cancer diagnosis and treatment.
• Develop and coordinate outpatient care transition and discharge plans based on patient needs.
• Collaborate with physicians, nurses, navigators and other staff to support care plans.
• Assist patients with access to financial aid, insurance resources, transportation, and community services.
• Coordinate referrals to behavioral health, hospice, home care, palliative care, and community resources.
• Support survivorship care plan development and regulatory compliance.
• Document assessments, interventions, care plans and outcomes in the EMR.
• Participate in multidisciplinary conferences and cancer program quality initiatives.
• Advocate for patients while maintaining confidentiality and ethical practice.
