Oncology Social Worker (LCSW) – Outpatient Oncology & Infusion Center

The Oncology Social Worker provides comprehensive psychosocial assessment, counseling, advocacy, and care coordination services to patients and families receiving treatment in an outpatient oncology and infusion center. Working as an integral member of the multidisciplinary cancer care team, this role supports patients throughout diagnosis, treatment, survivorship, and end-of-life care by addressing emotional, social, financial, and community resource needs. The Oncology Social Worker helps reduce barriers to care and promotes optimal patient outcomes, quality of life, and continuity of services.

Key Responsibilities
  • Conduct psychosocial assessments, distress screenings, and ongoing evaluations for oncology and infusion patients and their families.
  • Provide individual, family, and supportive counseling related to cancer diagnosis, treatment, survivorship, and adjustment to illness.
  • Develop and coordinate outpatient care transition and discharge plans based on patient needs, available resources, and clinical recommendations.
  • Collaborate with physicians, nurses, advanced practice providers, navigators, and other healthcare professionals to support patient-centered care plans.
  • Assist patients with access to financial assistance programs, insurance resources, transportation, community services, and supportive care programs.
  • Coordinate referrals to behavioral health, hospice, home care, palliative care, and other community-based resources as appropriate.
  • Support development and implementation of survivorship care plans consistent with oncology best practices and regulatory standards.
  • Document assessments, interventions, care plans, and outcomes in the electronic medical record.
  • Participate in multidisciplinary case conferences, quality initiatives, and cancer program committees.
  • Serve as a patient advocate while maintaining the highest standards of professionalism, confidentiality, and ethical practice.
Qualifications
  • Master's Degree in Social Work (MSW) from a CSWE-accredited program required.
  • Connecticut Social Work licensure required (LCSW preferred; LMSW considered based on experience and ability to obtain clinical licensure).
  • Minimum of two (2) years of social work experience in healthcare, oncology, infusion services, ambulatory care, or a related setting preferred. Relevant field placements may be considered.
  • Knowledge of psychosocial oncology, community resources, crisis intervention, care coordination, and patient advocacy.
  • Strong communication, counseling, assessment, and interdisciplinary collaboration skills.
