Job summary

The Ohio State University Wexner Medical Center is seeking a Patient Care Resource Manager (PCRM), a nurse case manager who guides patients through pre-admission to post-discharge coordination of care. The PCRM coordinates the patient plan of care, facilitates timely decisions, and works with physicians and nurses to move patients to the next level of care.

Required is a Bachelor’s degree; RN licensure; five years clinical experience preferred. A Master’s degree is preferred.

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• RN licensure required with five years clinical experience preferred.
• Knowledge of clinical practice guidelines and payer protocols desired.
• Experience with care management and nursing case management is implied.

Responsibilities

• Facilitate patient care through the health care system from pre-admission to post-discharge coordination.
• Coordinate the patient plan of care to enhance timeliness and efficiency of care decisions.
• Collaborate with physicians, nurses and other health care workers to manage transitions of care.
• Identify and remove barriers to progression of the care plan for each patient.
