Job summary

Salem Health is seeking an experienced Occupational Therapist in outpatient rehabilitation at Salem Hospital in Salem, OR. You will work with a diverse adult caseload and deliver one‑on‑one, evidence‑based interventions to empower patients to return to meaningful daily activities.

Responsibilities include evaluations, treatment planning, neuromuscular rehab, collaboration with an interdisciplinary team, patient education, and precise documentation.

Qualifications

• Holds a valid license to practice in Oregon or is license eligible.
• Basic Life Support (BLS) within 90 days of hire.
• Clinical experience in outpatient rehabilitation setting.

Responsibilities

• Provide thorough evaluations and create individualized treatment plans tailored to outpatient functional goals.
• Deliver goal‑oriented interventions focused on neuromuscular rehabilitation and cognition.
• Collaborate with PT, Speech, physicians, and care coordinators to support coordinated care.
• Educate patients and families on home programs and self‑management strategies.
• Document clearly and communicate professionally to support patient flow and outcomes.
