UPMC Passavant is hiring a full time Registered Nurse to support our Colorectal Surgery Clinic located within UPMC Passavant.

Schedule: M-F 7:30am - 4:30pm NO evenings, weekends or on call required. The Outpatient Nurse Coordinator provides direct patient care, oversees the unit, and coordinates care with physicians, homecare and ancillary staff to ensure comprehensive outpatient health management.

Responsibilities

• Participates in work that improves patient care and the professional practice environment. Provides oversight and supervision to the practice environment, directing clinical/administrative activity as needed. Demonstrates leadership with new nursing staff and office staff in daily patient management.
• Formulates goals and a plan of care for patients involving the patient as partner and considering individual needs holistically.
• Triages patient situations and coordinates care with the physician. Assists with all aspects of patient care including assessment, evaluation and education.
• Accountable for outpatient treatment follow-up necessary for patient health between visits. Serves as primary nursing resource to patient caseload.
• Facilitates necessary services outside the facility, interacting with homecare and other support services.
• Assists the departmental manager in patient care planning, operations and innovations.
• Becomes the patient advocate and voice to physicians and physician extenders; supports quality improvement and evidence-based practice.
