Responsibilities

• Demonstrates accountability for professional development that improves the quality of professional practice and patient care.
• Acts as a partner on the care team and supports coordination with physicians and staff.
• Triages patient situations and coordinates care with physician and clinic team.
• Assists physician with patient care activities including assessment, evaluation and education.
• Ensures outpatient treatment follow-ups are completed to maintain patient health between visits.
