Job summary

MEDICAL ON DEMAND LLC is seeking an experienced Advanced Practice Provider (APP) to join our outpatient team in Lady Lake, Florida. This role focuses on value-based care, preventive medicine, chronic disease management, and improving patient outcomes in a collaborative environment.

Open to NP or PA with Florida license, at least two years of clinical experience, and a passion for patient-centered care. Schedule is Monday–Friday with no nights or weekends.

Qualifications

• Active Florida license or ability to obtain
• Minimum of two years clinical experience
• Primary care experience preferred
• Value-based care or Medicare Advantage experience preferred

Responsibilities

• Provide outpatient primary care in a physician-led, value-based practice
• Manage preventive and chronic care for adults and seniors
• Build long-term patient relationships focused on quality outcomes
