Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Designs an appropriate plan of care with goals, outcomes and discharge plan.
• Educates patients and families using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Documents evaluations and plans of care per policy.
• Provides supervision to COTA and rehab technicians.
