Job summary

Beloit Health System is expanding its Behavioral Health team with a Social Worker and Outpatient Mental Health Therapist. The role focuses on assessment, counseling, and therapeutic interventions for individuals, families, and groups facing mental health and psychosocial challenges.

Qualifications

• Master's degree in Social Work, Counseling and Guidance or related degree from an accredited university.
• Licensed as LPC, LCSW or LMFT in Wisconsin (Minimal License is LPC-IT or MSW-IT).
• 3000 hours post-graduate experience.
• Strong clinical judgment, problem-solving, and crisis intervention skills.
• Ability to work independently while collaborating within an interdisciplinary team.
• Excellent organizational, documentation, and time management skills.
• Commitment to culturally competent and patient-centered care.
• Ability to maintain confidentiality and professional boundaries.
• Excellent verbal and written skills.
• Experience with EMR is a plus.

Responsibilities

• Conduct comprehensive psychosocial assessments and evaluations.
• Develop individualized treatment plans based on patient needs, goals, and clinical findings.
• Provide evidence-based individual, family, couples, and group therapy.
• Monitor patient progress and adjust treatment plans as appropriate.
• Assess and manage risk factors, including suicide ideation, self-harm, abuse, and crisis situations.
• Maintain timely and accurate documentation in EMR.
• Collaborate with psychiatrists, primary care providers, case managers, and other healthcare professionals to coordinate patient care.
• Educate patients and families regarding diagnoses, treatment options, coping strategies, and community resources.
• Participate in case consultations, team meetings, and quality improvement initiatives.
• Comply with all regulatory requirements, ethical standards, confidentiality guidelines, and organizational policies.
• Participates in an on-call rotation (1-2 weeks per year).
• Maintain knowledge of current behavioral health treatment practices and participate in continuing education opportunities.
