Job summary

Beloit Health System is seeking a Social Worker/Mental Health Therapist to join the Counseling Care Team on the 1st shift. The role focuses on psychosocial assessment, treatment planning, and delivering individual, family, and group therapy within an outpatient behavioral health setting.

The candidate will collaborate with psychiatrists, PCPs, and case managers while maintaining EMR documentation and participating in ongoing education. On-call duties are included periodically.

Qualifications

• Master's degree in Social Work, Counseling and Guidance or related field from an accredited university.
• Ideally licensed as LPC, LCSW or LMFT in Wisconsin (minimum LPC-IT or MSW-IT).
• Post-graduate experience: 3000 hours; ability to establish and maintain therapeutic relationships.
• Excellent clinical judgment, problem-solving, and crisis intervention skills.
• Ability to work independently while collaborating in an interdisciplinary team.
• Strong organizational, documentation, and time management skills.
• Commitment to culturally competent and patient-centered care.

Responsibilities

• Conduct psychosocial assessments and evaluations.
• Develop individualized treatment plans based on patient needs and clinical findings.
• Provide evidence-based individual, family, couples, and group therapy.
• Monitor patient progress and adjust treatment plans as needed.
• Assess and manage risk factors including suicide ideation, self-harm, abuse, and crises.
• Maintain timely documentation in the EMR.
• Collaborate with psychiatrists, primary care providers, and case managers.
• Educate patients and families about diagnoses, treatments, coping, and resources.
• Participate in consultations, team meetings, and quality improvement initiatives.
• Comply with regulatory standards, confidentiality, and policies.
• Participate in on-call rotation (1–2 weeks/year).
• Pursue continuing education and stay updated on best practices.
