Job summary

Bond Health in Brooklyn, NY is seeking a licensed Practical Nurse (LPN) for a full-time temporary assignment in an outpatient clinic. The role focuses on patient engagement, medication adherence, staff education, and day-to-day clinical coordination.

The position runs Monday–Friday, 9 AM–5 PM for 6–9 months. Requirements include an active NY LPN license and at least one year in ambulatory care; experience with EMR and MS Office is preferred. Bilingual English/Spanish is a plus.

Qualifications

• Active New York State LPN license.
• Minimum 1 year in ambulatory care or specialty outpatient clinics.
• Experience with EMR systems and patient databases.
• Strong communication, organization, patient engagement, and teamwork.

Responsibilities

• Provide patient-centered education to support appointments and medication adherence.
• Support PFH and other prevention, risk-reduction, sexual health, and substance-use education interventions.
• Coordinate staff training and refresher sessions on treatment-adherence.
• Collaborate with providers and case management to connect patients to services.
• Aid outpatient flow, scheduling, and daily team huddles.
• Document interactions and outcomes in EMR; track service delivery and quality.
