Job summary

Bond Health is seeking an experienced Licensed Practical Nurse (LPN) for a full-time temporary assignment in an outpatient clinic in Brooklyn, NY. The role supports patient engagement, medication and treatment adherence, staff education, and day-to-day coordination.

The ideal candidate will hold a NY LPN license, have at least 3 years of nursing experience including ambulatory or specialty outpatient settings, and be proficient with EMR systems and patient databases.

Qualifications

• Active New York State LPN license.
• Minimum 3 years of nursing experience.
• At least 1 year in ambulatory or specialty outpatient clinic setting.
• Experience with EMR systems and patient databases.
• Strong communication, organization, and teamwork skills.

Responsibilities

• Provide patient-centered education and follow-up to support medical appointments, medication adherence, and continuity of care.
• Coordinate staff training and refresher sessions; support delivery of adherence interventions.
• Work with providers and case management to connect patients with support services.
• Assist with outpatient patient flow, clinic scheduling, daily huddles, and follow-up outreach.
• Document patient interactions and outcomes in EMR.
• Maintain intervention records and support quality reporting.
