Job summary

Inova Health System’s Pulmonology department in Fairfax, VA is seeking a dedicated Licensed Practical Nurse (LPN) 2 to join our outpatient team. This full-time day position runs Monday–Friday, 8:00am–4:30pm, with the opportunity to contribute to care coordination and patient education.

You will administer prescribed medications, coordinate referrals, document care activities, and communicate with the healthcare team to support optimal patient outcomes.

Qualifications

• Education: Vocational / Technical Diploma in Practical Nursing.
• License: Licensed Practical Nurse in the Commonwealth of Virginia.
• Experience: 2 years of nursing practice in a hospital or clinic setting.
• Certification: BLS from the American Heart Association.

Responsibilities

• Coordinates referrals for patient chronic care conditions and follow up care as indicated by the initial care plan of the specialist and/or primary care physician.
• Administers prescribed oral and intramuscular medications, therapies and treatments following practical nursing standards and under the direct supervision of the physician.
• Effectively communicates with healthcare team members, patients and families; Supports healthcare team members to achieve team goals and assigns appropriate tasks to care assistants.
• Maintains and reviews patient medical records, notes and physician orders; Documents all patient care activities in accordance with documentation standards and legal requirements for medical facilities.
