Soleo Health is seeking a Licensed Practical Nurse (LPN) to provide essential support for Specialty Infusion Patients in Tempe, Avondale, and Glendale, AZ. This full-time position requires an LPN licensed in Arizona and offers a comprehensive set of benefits including competitive wages and mileage reimbursement.

The LPN will provide primary patient care, perform assessments and develop care plans while ensuring communication with healthcare team members.

Qualifications

• Current LPN license for applicable state in good standing.
• Must hold current IV certification in accordance with state and organizational standards.
• Minimum 3 years of nursing experience with background in infusion therapy preferred.

Responsibilities

• Provide direct care for assigned patients, including infusion therapy.
• Evaluate and educate patients/care partners on safe administration of infusion therapy.
• Develop and maintain caseload scheduling and communication.
