Job summary

Soleo Health Inc. seeks a Licensed Practical Nurse (LPN) to support infusion patients in Tempe, Avondale and Glendale, AZ.

The role focuses on direct patient care, assessments, care planning, and education in home or alternate site settings, following care matrices and best practices. You will work with the care team to educate patients and care partners, monitor IV therapy, and manage caseloads and scheduling.

Qualifications

• Current LPN license in AZ in good standing.
• IV certification current as per state/organizational standards.
• Minimum 3 years of nursing experience with infusion background preferred.
• Proficient with Microsoft Excel, Word and Outlook.
• Excellent customer service skills are required.

Responsibilities

• Provide direct care for assigned infusion patients and coordinate with pharmacists, physicians, dieticians, and home care staff.
• Educate patients and care partners on safely administering home infusion therapy and related instructions.
• Assess the home environment and implement safety/intervention measures for prescribed care.
• Develop and maintain each patient’s Care Map and evaluate response to therapy during visits.
• Discharge patients and complete discharge summaries within 7 days of discharge.
• Schedule and manage caseloads, updating weekly/monthly schedules and documenting visits.
• Document patient visits to physicians or labs in the calendar and EMR.
• Coordinate care via live or voicemail communications with the team.
• Document all communications at the time of service.
• Assist with vital signs, PICC dressing changes, PIV insertions, and administrative tasks as state law allows.
