Job summary

Software Placement Group in Coram, NY is seeking an Internal Medicine Nurse Practitioner for a 100% outpatient role with flexible hours Monday–Friday, part time or full time, no on-call. The NP will deliver direct outpatient primary care, including preventive, acute, and chronic disease management, with collaboration across care teams.

Requirements include BSN and MSN, NYS RN and NP licenses with BLS and board certification, plus 8+ years of NP experience and Internal Medicine training.

Qualifications

• BSN and MSN from an accredited nursing school.
• NYS RN and NP license, BLS, Board Certified.
• Training in Internal Medicine.
• 8+ years of experience as a NP.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented, patient-centered approach to care.
• Proven ability to maintain and grow a local clinical practice.
• Entrepreneurial mindset with ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.

Responsibilities

• Serve as a key clinical provider delivering high-quality patient care.
• Provide direct outpatient primary care, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Educate patients on diagnoses, treatment options, and preventive strategies.
• Maintain thorough, accurate documentation in the EMR system.
• Collaborate with referring physicians, specialists, and care teams for seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff.
• Maintain compliance with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and quality improvement projects.
• Contribute to building a sustainable patient panel aligned with practice growth goals.
