Qualifications

• BSN and MSN from an accredited nursing school.
• NYS RN & NP license, BLS, Board Certified.
• Trained in Internal Medicine.
• 8+ years experience as a NP.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care.
• Proven ability to maintain and grow a local clinical practice is required.
• Entrepreneurial mindset with an ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.

Responsibilities

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting.
• Refer patients to appropriate specialty services as needed.
• Provide communication and education to patients regarding diagnoses.
• Maintain thorough documentation within the Electronic Medical Record.
• Collaborate with referring physicians and specialists.
• Provide clinical oversight to nurses and support staff.
• Stay compliant with HIPAA, NYS DOH, CMS, and guidelines.
• Participate in population health initiatives.
• Contribute to the development of a sustainable patient panel.
