Job summary

Beacon Hill in Houston, TX seeks an Outpatient Counselor to provide individual and group counseling within a multidisciplinary team. The role focuses on intake, psychosocial evaluation, and personalized treatment planning in outpatient substance use services.

Responsibilities include counseling, case management, and coordination with community referrals. A Texas license (LCDC/LMSW/LPC/LMFT) is required and experience with EHR systems is preferred.

Qualifications

  • Active Texas LCDC, LMSW, LPC, or LMFT license in good standing.
  • Experience providing individual and group counseling services.
  • Knowledge of psychosocial assessments, treatment planning, and case management.
  • Ability to collaborate within a multidisciplinary treatment team while maintaining accurate clinical documentation.

  • Provide individual counseling, group counseling, and case management services for clients in outpatient treatment.
  • Develop, implement, and update individualized treatment plans based on client needs and progress.
  • Facilitate substance use education and interventions using approved curricula.
  • Monitor client progress, identify crises, and coordinate appropriate interventions and support.
  • Maintain accurate clinical documentation and records in EHR systems.
  • Participate in interdisciplinary team meetings, client case reviews, and community referrals.

Requirements
  • Active Texas LCDC, LMSW, LPC, or LMFT license in good standing.
  • Experience providing individual and group counseling services.
  • Knowledge of psychosocial assessments, treatment planning, and case management.
  • Ability to collaborate effectively within a multidisciplinary treatment team while maintaining accurate clinical documentation.
Job Duties
  • Conduct client intake assessments, orientations, and psychosocial evaluations.
  • Provide individual counseling, group counseling, and case management services for clients participating in outpatient treatment programs.
  • Develop, implement, and update individualized treatment plans based on client needs and progress.
  • Facilitate substance use education and therapeutic interventions using approved treatment curricula.
  • Monitor client progress, identify potential crises, and coordinate appropriate interventions and support services.
  • Maintain accurate, timely clinical documentation and records within electronic health record systems.
  • Participate in interdisciplinary team meetings, client case reviews, and community referral coordination to support continuity of care.
Additional Information

Schedule: Tuesday through Friday 8:30am to 4:30pm Saturday 8am to 4pm

Compensation:Up to $50,000/annually

Preferred Experience:

  • Experience utilizing electronic health records and clinical documentation systems.
  • Strong skills in assessment, crisis intervention, and treatment planning.
  • Bachelor's degree in Social Work or a related field preferred.
  • Valid Texas driver's license and ability to meet insurability requirements
