JobRx, Inc. is looking for a Community Pharmacist in Glenwood Springs, Colorado. In this role, you'll support patients by enhancing medication safety during hospital admissions and discharges.

Your responsibilities include collaborating with care teams, balancing patient needs, and serving as a Medication Reconciliation Pharmacist. Candidates must have a BS or PharmD, a Colorado pharmacy license, and be immunization certified.

Qualifications

• Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD degree.
• Licensed to practice pharmacy in Colorado (or must obtain within 60 days of hire).
• Immunization certified with active BLS card.

About the Role:

We are seeking a Community Pharmacist to join our team. This role supports patients by improving medication safety during hospital admission, discharge, and in daily life.

Responsibilities:

• Work in the outpatient pharmacy and collaborate with care teams to fulfill hospital discharge orders.
• Balance hospital discharge needs with the needs of community patients.
• Rotate as a Medication Reconciliation Pharmacist with other pharmacy staff.
• Collaborate closely with inpatient and outpatient pharmacy teams and healthcare providers.

Work Schedule:

• Monday-Friday: 7:00 AM - 5:30 PM.
• Saturday: 8:00 AM - 4:00 PM.
• Closed Sundays.
• Rotating holidays and Saturdays.

Qualifications:

• Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD degree.
• Licensed to practice pharmacy in Colorado (or must obtain within 60 days of hire).
• Immunization certified with active BLS card.

Skills:

Experience:
