Job summary

Intermountain Health St. Mary’s Regional Hospital in Grand Junction, Colorado, seeks an Outpatient Clinical Pharmacist for a PRN role with hours as needed, Monday through Friday, 8:00 AM to 6:00 PM.

This is a unique opportunity to help launch a new outpatient pharmacy and shape patient-centered workflows. You will partner with the Pharmacy Manager, pharmacists, and technicians to deliver safe, seamless care while developing operations, processes, and standards for a growing program.

Qualifications

• Doctor of Pharmacy or PharmD or BS in Pharmacy from an accredited institution.
• Current license to practice pharmacy in the practice state.
• Immunization certification if applicable to practice site needs.
• Knowledge of state and federal regulations for pharmacy practice.

Responsibilities

• Reviews medications for allergies and interactions and optimizes therapy.
• Ensures timely preparation and delivery of medications to care areas.
• Provides patient counseling and education on medications.
• Supervises interns and technicians and offers clinical consultation.
• Identifies safety hazards and opportunities to improve patient care.
• Participates in departmental meetings and process improvement.
