Job summary

Qualifications

• RN required (licensure)
• Minimum 2 years nursing experience; Bachelor's degree preferred with 2–5 years of experience
• Licensure as RN
• Outpatient or adult care experience preferred

Responsibilities

• Assess patient preferences, barriers and needs, including emotional, spiritual, cultural, and population-specific factors.
• Develop, implement and document individualized care plans collaboratively with the healthcare team and patients' families or caregivers, ensuring effective communication across care settings.
• Deliver respectful, equitable and empathetic care to diverse populations, supporting emotional and cultural preferences.
• Participate in professional development activities, community health events and quality improvement initiatives to advance nursing practice.
• Evaluate patient condition changes, collaborate with families and the team, and update care plans accordingly, ensuring evidence-based practice and quality outcomes.
• Work in a safety-sensitive role requiring alertness and safe practice.
