Job summary

Intermountain Children's Home in Helena, MT is seeking an Outpatient Case Manager to support children and families within our Direct Care team. You will coordinate referrals, assessments, treatment planning, conferences, and discharge planning to promote successful outcomes.

You will act as the primary liaison among families, guardians, treatment staff, and community partners, ensuring clear communication and timely documentation in compliance with Medicaid and regulatory requirements.

Qualifications

  • Bachelor’s degree in social science or closely related field or equivalent experience.
  • 19 years or older
  • Valid Driver’s License
  • Background check before start date

Responsibilities

  • Serve as liaison between families, guardians, partners, and the treatment team to coordinate care.
  • Coordinate referrals, admissions, treatment planning, conferences, and discharge planning.
  • Collaborate with families and providers to support treatment goals and address barriers.
  • Provide families with education, resources, and support to improve engagement.
  • Maintain accurate documentation and ensure compliance with agency and Medicaid requirements.
  • Monitor progress and support continuity of services throughout the child’s treatment.

To be successful in this role, you will:

Qualifications:

Required

  • Graduation from an accredited college or university with a bachelor’s degree in social science or closely related field or equivalent experience.
  • 19 years or older
  • Valid Driver’s License
  • Must pass a Background check prior to start date
