Sentara Health seeks a Physician Practice and Clinic Registered Nurse in Chesapeake, VA to provide nursing care in ambulatory settings. You will assess patients, assist with exams, administer medications and immunizations, and educate patients while coordinating care with physicians and office staff.

You will manage triage, follow-up, and patient communication, support population health outreach, and document care in the EHR, contributing to safe, efficient clinic operations.

Qualifications

• ADN required; BSN preferred.
• 1–2 years in ambulatory/clinic or medical-surgical nursing preferred.
• Current state RN license; NCLEX-RN completed; BLS required; AMB-BC certification preferred.

Responsibilities

• Assesses patients and performs triage in person and by phone.
• Assists physicians with examinations and procedures.
• Administers medications, immunizations, and treatments.
• Provides patient education and self-management support.
• Coordinates referrals, follow-up, and continuity of care.
• Manages patient messages and results within scope.
• Supports prevention and population health outreach.
• Maintains a safe, clean, and stocked clinical environment.
• Documents care in the electronic health record.
• Performs other related duties as assigned.
