• Licensed to practice in Pennsylvania as an LCSW, LPC, or LMFT.

Responsibilities

• Assesses psychosocial needs of patients and families/caregivers.
• Conducts thorough reviews of medical records and referring documentation.
• Assists in development and administration of treatment plans and behavior plans.
• Monitors progress and adapts treatment efforts with patient and team.
• Maintains patient confidentiality and coordinates with the treatment team.
