Job summary

UPMC Chautauqua is seeking a Service Coordinator for Outpatient Addiction Medicine. This full-time, day-shift role focuses on coordinating mental health services for individuals with serious diagnoses to support recovery in relationships, housing, and employment.

The position emphasizes recovery-oriented care within a collaborative team, with opportunities for career growth within the respected UPMC system and a stable schedule with no weekends or holidays.

Qualifications

• Bachelor's degree in a Human Services field such as Social Work, Psychology or Counseling.
• Other fields may be considered with at least 12 credits in Human Services.
• One-year related experience preferred.
• Safe driving record required.
• BLS or CPR certification and Driver's License required.

Responsibilities

• Provides professional coordination of mental health services according to the individual's level of need within a treatment/service team, often taking the lead to advocate for recovery.
• Assesses strengths and needs, develops and documents a comprehensive service plan guided by the individual’s goals and needs.
• Maintains regular training, hospital/transition contact, and timely records of Service Coordination activity; participates in on-call rotations.
