Job summary

The ACT Psychiatric Nurse Practitioner at Southwest Network serves as the psychiatric medical provider and senior prescribing clinician on the ACT team, delivering medication management, clinical leadership, and care coordination for members in community settings.

This outpatient position requires board eligibility, experience with severe mental illness (SMI), and a commitment to collaborative, person-centered care, with after-hours coverage and on-call responsibilities as needed.

Qualifications

• Board-certified or eligible PMHNP required.
• Experience with serious mental illness (SMI) populations preferred.
• Strong collaboration and communication skills with multidisciplinary teams.

Responsibilities

• Conduct comprehensive psychiatric assessments and treatment planning.
• Provide ongoing psychopharmacologic management including clozapine and LAI antipsychotics.
• Incorporate brief therapy during visits when appropriate to enhance outcomes.
• Consult with colleagues on complex or treatment-resistant cases.
• Provide psychiatric care for Court Ordered Treatment (COT) cases as part of caseload.
• Coordinate with primary care, pharmacy, and inpatient teams during care transitions.
