Job summary

Southwest Network, Inc. is seeking a Psychiatric Nurse Practitioner (ACT Team) for our outpatient services in Phoenix, AZ. The role focuses on leading medical services for the ACT team, with a defined caseload and after-hours coverage as part of the team-based care model.

You will perform comprehensive psychiatric assessments, manage medications (including clozapine and LAIs), and may integrate brief therapy to maximize outcomes. Collaboration with primary care and other teams is essential.

Qualifications

• Two years of experience as a psychiatric nurse practitioner (PMHNP).
• Valid DEA certification.
• Arizona license as an NP or eligibility for licensure.

Responsibilities

• Perform comprehensive psychiatric assessments and treatment planning.
• Provide ongoing psychopharmacologic management including complex regimens.
• Integrate brief therapy during visits when appropriate to enhance outcomes.
• Coordinate with primary care, pharmacy, and inpatient teams during transitions.
