Job summary

Virtua Health in New Jersey seeks Per Diem Scheduling & Coverage professionals (RN or MSW) to support five hospital campuses. Role requires weekday & weekend availability, 8:30 a.m. to 5:00 p.m., and participation in special days each year.

You will coordinate patient care with physicians and multidisciplinary teams, ensuring safe transitions across the care continuum while maintaining compliance and quality standards. This is a part-time, on-site Per Diem position with competitive hourly pay.

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum of 1 year Case Management or other interdisciplinary experience including Homecare Coordination in an Acute Care setting; understanding of Medicare, Medicaid, managed care and discharge planning.
• Excellent verbal and written communication skills, problem solving, critical thinking and conflict resolution.
• Required Education: RN or Masters prepared Social Worker.
• Training / Certification / Licensure: RN or LSW required; LCSW considered.

Responsibilities

• Care Coordination - coordinates patient care with the goal of high-quality, cost-effective care.
• Acts as a clinical expert and keeps up with clinical practice guidelines.
• Engages physicians to ensure seamless continuity of care and act as the single resource.
• Documentation - accurate and complete EMR and case management documentation.
• Participates in organizational improvement activities and quality initiatives.
• Identify and report compliance issues as appropriate.

Job Information: Per Diem Scheduling & Coverage Requirements: This position requires the flexibility to support all five hospital campuses: Voorhees, Marlton, Camden, Willingboro, and Mt. Holly. Candidates must have weekday & weekend availability from 8:30 a.m. to 5:00 p.m. In addition, Per Diem Outcomes Managers are required to work two special days per year. Special days include Easter, Mother's Day, Father's Day, Halloween, and the day before or after a major holiday (New Year's Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, and Christmas).

Job Description Summary

Responsible for partnering with the physician and multidisciplinary team in coordinating patient care with the underlying objective of enhancing quality and cost effective care. Acts as a resource and patient advocate to facilitate a safe transition across the continuum.

Position Responsibilities

• Care Coordination - completes appropriate assessments and plans effectively to meet patient needs, monitors the length of stay and promotes efficient utilization of resources.
• Acts as a clinical expert and maintains ongoing knowledge of clinical practice guidelines.
• Engages the physician and builds seamless continuity of care and is the physicians single consistent resource.
• Problem resolution, patient/family communication.
• Helps identify factors impeding patient progression, resolves, escalates and documents as appropriate.
• Refers appropriate cases for Social Work intervention.
• Documentation - Appropriate and complete documentation of individual plan of care in EMR and case management documentation system.
• Metrics- Accountable to job specific goals, objectives and dashboards which contribute to the success of the organization.
• Participates in organizational improvement activities including patient satisfaction, Six Sigma committee, department and/or divisional teams and community activities.
• Compliance - understands and applies applicable federal and state requirement.
• Identify and reports compliance issues as appropriate.

Position Qualifications

• Required Experience: Minimum 3 years clinical nursing (RN) experience or Social Work
• Minimum of 1 year Case Management or other interdisciplinary experience including Homecare Coordination in an Acute Care setting; understanding of Medicare, Medicaid, managed care and discharge planning. (3-5 years preferred)
• Excellent verbal and written communication skills, problem solving, critical thinking and conflict resolution.
• Required Education: RN or Masters prepared Social Worker
• Training / Certification / Licensure: RN or LSW required; LCSW considered

Hourly Rate: $38.33 - $59.58

The actual salary/rate will vary based on applicant's experience as well as internal equity and alignment with market data.
