• UDSMR PPS Certification within 3 years.

Texas Rehabilitation of Plano is a brand‑new 40‑bed inpatient rehabilitation facility opening in 2026. We offer high‑quality, compassionate care for a wide variety of patients in need. Our teams provide evidence‑based, transformational care for a broad range of rehabilitation concerns in a comfortable environment.

How You’ll Contribute

• Responsible for staff orientation and ongoing education to improve QI score accuracy at admission, throughout the stay, and upon discharge.
• Serve as educator for Quality Indicators (QI) scoring according to Uniform Data System (UDS) training materials.
• Act as a resource to licensed clinicians and clinical staff regarding questions and interpretation of QI scoring guidelines.
• Provide ongoing education to ensure accurate and timely completion of the IRF‑PAI.
• Facilitate biannual UDSMR staff credentialing for QI scoring.
• Review medical record documentation, including UDS Pro‑Doc, to support accurate and timely functional scores and comorbid conditions per CMS guidelines.
• Ensure the IRF‑PAI reflects functional status and 24‑hour care needs.
• Collaborate with physicians and interdisciplinary team members to capture accurate assessments of patients’ needs and strengths.
• Evaluate accuracy of facility clinical documentation using QI Manager.

What We’re Looking For

• Education: Bachelor of Science in Nursing, Associate Degree in Nursing, Nursing Diploma, or Licensed/Registered PT, OT, SLP.
• License: Current unrestricted state licensure as Registered Nurse (preferred) or PT, OT, SLP. CRRN preferred. CPR/BCLS certification. UDSMR PPS Certification within 3 years.
• Experience: 3+ years of experience in acute hospital or rehabilitation setting.
• Additional requirements: Excellent oral and written communication and interpersonal skills.
