Job summary

VitalCore Health Strategies in Delaware is seeking an OUD Reentry Specialist at the Howard R. Young Correctional Institution in Wilmington for a full-time position.

You will provide case management, reentry planning, and transitional support to individuals with substance use disorders and co-occurring needs, coordinating with correctional staff, treatment providers, and community agencies. The role requires a Bachelor’s degree in a related field and at least two years of experience with

Qualifications

• Bachelor's degree in a relevant field required.
• Minimum of two years of experience working with justice-involved individuals, substance use treatment, mental health services, case management, or reentry services.
• Knowledge of Opioid Use Disorder, Medication-Assisted Treatment (MAT), mental health disorders, trauma-informed care, and recovery-oriented practices.
• Experience coordinating services with community agencies and healthcare providers.
• Strong interpersonal, communication, and organizational skills.
• Ability to establish rapport with diverse populations and maintain professional boundaries.

Responsibilities

• Provide case management, reentry planning, and transitional support services to incarcerated individuals with substance use disorders, mental health needs, and co-occurring disorders. Work collaboratively with correctional staff, treatment providers, community agencies, and other stakeholders to ensure access to services and supports for successful reintegration.
