Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Manitowoc, WI. The position is Part Time or PRN, focused on delivering occupational therapy services according to the facility’s plan of care and applicable standards.

The ideal candidate will hold an Associate's degree in occupational therapy, possess an active OTA license in the practice states, and commit to ongoing professional development to maintain licensure.

Qualifications

• Associate's degree in occupational therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings to determine the best intervention for each resident.
• Implement treatment plans and state goals with supervision from a licensed therapist.
• Communicate evaluation findings and recommendations to residents, families, and the team.
• Schedule resident therapy in coordination with team members.
• Document and bill occupational therapy services according to the plan of care.
• Perform additional duties to support resident care as assigned.
• Adhere to CMS documentation standards and daily charting requirements.
