Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Wisconsin. This full-time or PRN position focuses on planning, directing, and delivering occupational therapy services to residents within current guidelines and CMS standards.

The OTA will screen residents, develop and follow treatment plans, and collaborate with an interdisciplinary team to ensure high quality resident care.

Qualifikationen

• Must have an Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Aufgaben

• Conduct regular screening of residents to determine the best intervention.
• Implement treatment plan and follow goals established by supervising therapist.
• Interpret and communicate evaluation findings and plans to residents and families.
• Establish resident therapy schedules in cooperation with team members.
• Document and bill occupational therapy services per established plan.
• Perform other duties to effectively treat patients as required.
• Comply with CMS documentation standards and daily charting.
