Job summary

Evolve Therapy Services is hiring compassionate Occupational Therapy staff for an in-house Skilled Nursing Facility in Greendale, WI. The Certified Occupational Therapy Assistant will plan, organize, develop, and direct Occupational Therapy services in accordance with federal and state standards to maintain high-quality resident care.

The role involves screening residents, executing treatment plans, communicating evaluations to families and the care team, and coordinating therapy schedules.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans and progress toward goals set by supervising therapist.
• Interpret and communicate evaluation findings to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point of care documentation.
