Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant to join an in-house skilled nursing facility in Johnstown, PA. The OTA will plan, direct and deliver occupational therapy services to residents in collaboration with supervising therapists and the interdisciplinary team.

Responsibilities include screening residents, implementing treatment plans, documenting progress, coordinating schedules, and ensuring CMS-compliant documentation.

Formación

• Associate's degree in Occupational Therapy demonstrates foundational therapy knowledge.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Screen residents to determine best intervention per scope of practice.
• Perform treatment plans and follow goals established with supervision.
• Communicate evaluation findings to residents, families, and team members.
