Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. You will plan, direct, and provide OT services to residents under supervision, following approved plans of care and CMS guidelines.

Qualifications

• Active OTA licensure in state(s) of practice
• Ongoing continuing education to maintain licensure and certification
• Ability to plan and direct Occupational Therapy services in SNF setting

Responsibilities

• Conduct regular screening of residents to determine best interventions
• Perform treatment plans and follow short- and long-term goals
• Interpret and communicate evaluation findings to residents and team
• Establish individual resident therapy schedules with team
• Conduct therapy per treatment plan and ensure documentation and billing
• Perform other duties as required to treat patients
• Comply with CMS documentation standards daily
