Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. The role supports resident care with planned, directed OT services and aligns with federal and state standards.

Responsibilities include evaluating residents, implementing treatment plans, coordinating therapy schedules with the team, and ensuring proper documentation and billing. Requires active OTA licensure and ongoing professional development.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Implement treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, plans, and recommendations to residents and families.
• Establish individual resident therapy schedules in cooperation with the team.
• Deliver therapy according to the treatment plan and document/bill services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point-of-care documentation.
