Job summary

Sagewood is seeking an Occupational Therapist to join our in-house team serving Assisted Living, Skilled Nursing, and Memory Care residents in Phoenix. The role is PRN with flexible scheduling and a focus on delivering high-quality therapy to a senior population.

Responsibilities include evaluating patients, developing treatment plans, documenting progress, and coordinating care with other staff while maintaining a clean, safe environment and strong resident relations.

Qualifications

• Bachelor’s degree in Occupational Therapy required.
• Licensed by the State in the designated geographic area.
• CPR certification required.

Responsibilities

• Maintain and ensure accuracy of patient care records for all patients on personal caseload.
• Report billing information for Occupational Therapy for all patients on personal caseload.
• Provide direct patient care as prescribed, including evaluation and treatment planning.
• Report and interpret treatment results.
• Participate in development/in-services for the facility as requested.
• Supervise support personnel in immediate work areas as appropriate.
• Perform clinical functions: screens, evaluations, treatment plan development, and delivery of treatment.
• Coordinate with other staff to carry out the treatment plan.
