Job summary

Sagewood is seeking an Occupational Therapy Assistant to join our in-house team providing care across Assisted Living, Skilled Nursing, and Memory Care units in Phoenix. This PRN role supports an active patient population at both outpatient and SNF settings with a focus on compassionate elder care.

The pay rate ranges from $30 to $38 per hour. The position offers flexible scheduling to promote work-life balance and opportunities for professional growth within a respected senior living operator.

Qualifications

• Graduation from an approved and accredited therapist program.
• CPR certification and state licensure required.
• Ability to perform sedentary tasks with some physical activity.

Responsibilities

• Maintain and ensure accuracy of patient care records for all patients on your caseload.
• Accurately report billing information for Occupational Therapy for your patients.
• Assist the registered Occupational Therapist in carrying out procedures and programs.
• Perform routine treatment procedures per the planned program.
• Adapt treatment approaches and environment to patient needs under supervision.
• Report pertinent findings to the supervising therapist.
• Participate in in-service development for the facility as requested.
• Deliver treatment following facility, Medicare, and policy procedures.
