Job summary

• Evaluate each patient's functional abilities and needs, then develop individualized therapy plans in coordination with physicians and the care team.
• Deliver skilled, evidence-based interventions that promote healing, independence, and quality of life.
• Monitor patient progress, adjust plans as needed, and ensure thorough documentation.
• Educate patients and families to build understanding, confidence, and engagement throughout recovery and beyond.
• Collaborate with interdisciplinary partners to ensure seamless transitions of care and exceptional patient experiences.
• Lead by example and uphold HCA Healthcare's mission and values.
• Responsible for clinical management of the patient.
• Ensure satisfaction of customers (patients, visitors, physicians and staff) with ICARE values.
• Participate in and understand Quality Management activities.
