Concept Rehab, Inc. in Willoughby, OH is seeking an Occupational Therapist for a Part-Time or PRN position at our Post-Acute Rehab/SNF.

You will evaluate, treat, and supervise patients to improve mobility, cognition, strength, and activities of daily living in accordance with AOTA guidelines. You will collaborate with families, physicians, and the interdisciplinary team, document services daily, and supervise OT/COTA students while supporting discharge planning and adaptive equipment

Formación

• Required Master’s or Doctorate in Occupational Therapy.
• Active Occupational Therapist license in Ohio.
• Strong communication and documentation skills.

Responsabilidades

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician orders.
• Develop and implement effective, individualized treatment plans.
• Supervise COTAs in clinical delivery.
• Communicate progress with physicians, clinical managers, patients, and family members.
• Participate in care meetings and interdisciplinary team discussions.
• Document services accurately, completing daily billing and treatment notes per policy.
• Instruct families and nursing staff on patient maintenance and restorative programs.
• Assist with discharge planning and recommend adaptive equipment.
• Supervise OT/COTA students from affiliating universities and colleges.
