Job summary

Concept Rehab & Engage Consulting is seeking a dedicated Occupational Therapist (OT) for a Part-Time or PRN role in a Post-Acute Rehab/SNF setting in Willoughby, OH. You will evaluate, treat, and supervise patients to improve mobility, cognition, strength, and ADLs in collaboration with families, physicians, and the care team.

Responsibilities include rapid patient evaluation after orders, treatment planning, supervising COTAs, and discharge planning.

Qualifications

• Participate in care meetings and interdisciplinary team discussions.
• Document services, completing daily billing and treatment notes per policy.
• Instruct families and nursing staff on patient maintenance and restorative programs.
• Assist with discharge planning and recommend adaptive equipment.
• Supervise OT/COTA students from affiliating universities.

Position Summary

We are seeking a dedicated Occupational Therapist (OT) for a Part-Time or PRN position at our Post-Acute Rehab / Skilled Nursing Facility (SNF). In this role, you will evaluate, treat, and supervise patients to improve mobility, cognition, strength, and activities of daily living (ADLs). You will work in accordance with AOTA guidelines and collaborate closely with families, physicians, and the interdisciplinary care team.
