Midtown Oaks Post Acute in Sacramento, CA is seeking a Full-Time Occupational Therapist to evaluate residents, develop individualized treatment plans, and help patients regain independence in daily activities.

Join our collaborative rehabilitation team and provide therapy to skilled nursing and post-acute residents, with a competitive hourly rate of $60–$65 and a $7,500 sign-on bonus. California OT licensure and an accredited OT program are required.

Qualifications

• Active California OT license required.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within an interdisciplinary team.
• Preferred SNF/post-acute/ LTC experience.
• Experience with geriatric populations.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled occupational therapy services to improve function, mobility, and independence.
• Assess ADLs and implement therapeutic interventions.
• Collaborate with physicians, nursing staff, therapists, and families to optimize outcomes.
• Educate residents and caregivers on treatment plans and adaptive equipment.
• Monitor progress and modify treatment plans as needed.
• Complete accurate documentation in accordance with regulations.
• Maintain compliance with professional and ethical standards.
