Job summary

Adoration Health, through its Home Health division Adoration Home Health, is seeking a dedicated Home Health Occupational Therapist to serve Charlottesville, VA. You will provide patient-centered OT services in a home setting, coordinating with families and physicians.

This full-time role offers a Monday–Friday schedule with potential 32-hour weeks, 1:1 patient care, and opportunities for professional growth. Home health and HCHB experience are preferred.

Qualifications

• Graduate of a CAPTE-approved Occupational Therapy Education program.
• Current OT licensure in state and CPR certification required; CPR may be offered at hire.
• Two years of experience in adult OT; home health preferred.
• Excellent observation and communication skills; problem-solving and math basics.

Responsibilities

• Perform OT evaluation and treatment per physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans for independence and function.
• Maintain communication among the treatment team, physician, patient and family.
• Document services thoroughly for accurate billing.
• Supervise assistants in direct patient care per state practice act.

Responsibilities

As a Home Health Occupational Therapist, You will:

• Perform occupational therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Occupational Therapy Assistants in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a Occupational Therapy Education program approved by CAPTE
• Current Occupational Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult occupational therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
