Adoration Health is seeking a Home Health Occupational Therapist to provide 1:1 patient care in Colonial Heights, VA. You will evaluate and treat patients per physician orders and help them reach maximum independence at home.

The role emphasizes collaborative care with a supportive team, flexible scheduling, and opportunities for growth within a growing home-health organization.

Qualifications

• Graduate of a CAPTE-approved OT program.
• Current OT licensure in state and CPR certification.
• Two years of adult OT experience; home health preferred.
• Excellent observation, verbal and written communication skills; problem solving.

Responsibilities

• Perform OT evaluation and treatment per physician orders, including adaptive equipment recommendations.
• Establish measurable goals and treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
• Maintain thorough documentation for proper billing.
• Supervise OT Assistants in direct patient care and related activities as allowed by state practice act.

Coverage area: North territory

Responsibilities

As a Home Health Occupational Therapist , You w ill:

• P erform occupational therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Occupational Therapy Assistants i n direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a n Occupational Therapy Education program approved by CAPTE
• Current Occupational Therapist licensure in State and American Heart Association CPR certification required ; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult occupationa l therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
