Lodi Nursing and Rehabilitation in Lodi, CA is hiring a licensed Occupational Therapist (OT) for full-time, on-site work. You will evaluate, plan, and implement individualized OT care, supervise OTAs, and lead discharge planning to promote residents’ independence and safety in a Skilled Nursing setting.

The role requires active CA OT licensure, NBCOT, and CPR/BLS, with a focus on ADLs/IADLs, upper-extremity rehab, and outcome-driven care. Competitive compensation and full benefits offered.

Formación

• Current, active California OT license — required (pending licensure considered)
• Graduate of an accredited Occupational Therapy program
• NBCOT certification — required
• Current CPR/BLS certification
• Continuing education compliance with California OT Board
• Experience in skilled nursing or post-acute care preferred
• Excellent verbal and written communication and clinical judgment
• Team-first mindset with ability to supervise OTAs and aides

Responsabilidades

• Conduct comprehensive occupational therapy evaluations and re-evaluations per physician orders and California standards
• Develop, implement, and modify individualized plans of care with measurable goals
• Deliver skilled interventions addressing ADLs, IADLs, upper extremity rehab, cognitive retraining, and sensory integration
• Assess and recommend adaptive equipment, devices, splints, and environmental modifications
• Supervise and direct Occupational Therapy Assistants and rehabilitation staff
• Maintain timely, accurate documentation per federal/state/payer requirements
• Lead discharge planning and care transition coordination
• Communicate findings and progress to physicians, families, and interdisciplinary team
• Ensure HIPAA compliance and professional standards
