Job summary

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy that is recognized by the AOTA.
• SNF experience preferred
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide an OT evaluation based on MD orders.
• Document findings on the evaluation form promptly and accurately.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify as indicated with precautions.
• Document patient progress daily and ensure timely progress notes.
• Update treatment plan monthly on recertification.
• Document discharge on a discharge summary as indicated.
• Document caregiver or discharge in-services as indicated.
• Complete any and all other documentation forms required by Facility.
• Document care plans and MDS information as needed by the facility.
• Attend facility and rehab meetings as indicated by Supervisor/Regional Manager.
• Participate in facility committee meetings as indicated.
• Act as a rehab delegate to the interdisciplinary team.
• Participate in scheduling of patients as indicated.
• Perform screens as indicated.
• Complete billing and bill patients for occupational therapy services rendered.
• Cover other therapists' caseload during absence as necessary.
• Supervise junior staff, students and technicians; co-sign notes as indicated.
• Maintain professional relationships with staff, patients and families.
• Provide in-service education regarding patient care or occupational therapy services.
• Comply with all facility policies and patient confidentiality and rights.
• Report to work on time, adhere to hours and project a professional image.
• Adhere to productivity requirements.
• Follow HR policies re: lunch, overtime, sick and time off notification.
• Maintain a positive work atmosphere, fostering good relations.
• Perform any other duties assigned by supervisor or Regional Manager.
• Adhere to all facility policies.
