Job summary

The Judge Group is seeking a PRN Home Health Occupational Therapist to provide skilled OT services to patients in the Greater Philadelphia region. You will evaluate patients in their homes, develop personalized plans, and focus on ADLs, mobility, safety and independence, while coordinating with a multidisciplinary team.

Ideal candidates hold a Pennsylvania OT license, have home health experience if possible, and can work independently with reliable transportation.

Qualifications

• Active Pennsylvania Occupational Therapist license.
• Home Health experience preferred.
• OASIS experience preferred.
• Strong assessment and patient education skills.
• Ability to work independently in a community-based setting.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete occupational therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans.
• Provide therapy focused on ADLs, functional mobility, strength, coordination and safety.
• Assess home environments and recommend adaptive equipment or home modifications.
• Educate patients and caregivers on home safety and fall prevention strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RNs, PTs, SLPs, social workers and other care team members.
• Complete accurate and timely EMR documentation.

Position Overview

A Medicare-certified, Pennsylvania-licensed home health agency is seeking a PRN Occupational Therapist (OT) to provide skilled therapy services to patients in their homes throughout the Greater Philadelphia region.

Patients are typically recovering from surgery, hospitalization, illness, injury, or disability, with a focus on maximizing independence and safety in the home environment.

Responsibilities

• Complete occupational therapy evaluations in patients' homes
• Develop and implement individualized treatment plans
• Provide therapy focused on ADLs, functional mobility, strength, coordination, and safety
• Assess home environments and recommend adaptive equipment or home modifications
• Educate patients and caregivers on home safety and fall prevention strategies
