Job summary

Deer Meadows Home Health is seeking a dedicated Occupational Therapist (OTR/L) for a Full-Time or PRN opportunity in Home Health and Community-Based settings, including Assisted Living, Independent Living, and Personal Care. This role offers the chance to provide one-on-one care across a diverse population while collaborating with an interdisciplinary team to improve independence and quality of life.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Pennsylvania OT license (OTR/L) or eligibility.
• Home Health experience preferred.
• Experience with HCHB is a plus.
• Strong clinical judgment, communication, and organizational skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled interventions to improve ADLs, safety, and functional mobility.
• Deliver therapy services in homes and community-based settings (ALF, IL, Personal Care).
• Educate patients and caregivers on adaptive equipment and home safety.
• Collaborate with nurses, therapists, and interdisciplinary team members.
• Complete timely documentation in HCHB EMR.
• Independently manage caseload and daily schedule.
