Job summary

Therapy Management Corporation is seeking an Occupational Therapist to provide adult/geriatric rehabilitation services. You will evaluate patients, develop personalized care plans, and deliver patient-centered therapy to improve independence and quality of life.

You’ll collaborate with interdisciplinary teams, maintain clinical documentation, and emphasize autonomy while delivering high-quality care in a supportive environment.

Qualifications

• Must hold a valid Occupational Therapist license (or be eligible) in the practicing state.
• Graduate of an accredited OT program.
• Passion for patient-centered care and improving quality of life for older adults.
• Experience or interest in skilled nursing, long-term care, or geriatric rehab.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life.
• Deliver care through therapeutic activities, ADL training, and adaptive techniques.
• Help patients improve daily tasks such as dressing, bathing, grooming, and meal preparation.
• Partner with interdisciplinary teams, caregivers, and nursing staff to support patient outcomes and continuity of care.
• Track progress, maintain clinical documentation, and adapt treatment plans.
