Job summary

Advocate Health is seeking an Occupational Therapist for the Acute Care Rehabilitation unit in Durham. The role involves evaluating patients, developing and implementing individualized treatment plans, and coordinating discharge instructions with family training.

Qualifications

• BLS for Healthcare Providers within 6 months (unless exempt).
• State license as Occupational Therapist (as applicable).

Responsibilities

• Interprets physician referrals and evaluates patient abilities and how they impact daily living.
• Establishes a plan of care and implements treatment; fabricates assistive devices as needed; monitors outcomes.
• Plans for and completes patient discharge with home programs and community referrals.
• Completes required documentation including progress notes, discharge summaries, and scheduling; applies insurance guidelines.
• Cleans up after treatment and maintains orderly space; assists with positioning, transfers, and safe patient handling.
• Demonstrates use of modalities and equipment; ensures safety and knowledge of indications/contraindications.
• Participates in performance improvement activities and supports department coverage needs.
• Pursues professional education and orients new staff; participates in coworker evaluations.
